HEALTH CONNECTIONS MENDIP
Health Connections Mendip is a new health and wellbeing service available to patients. Patients can talk to a health connector at the surgery to find out about local activities, support and services available, and can also arrange one-to-one appointments to speak to a Connector. During these appointments a Health Connector will listen to your health story and assist you with setting health-related goals which are meaningful to you. Health Connectors also support you to make positive changes that last. One-to-one appointments with a Health Connector at Wells Health Centre
Health Connectors support people to make positive changes to improve their health and wellbeing. Health Connectors can listen to your health story and give you information about local services such as exercise classes and support groups. They also work one-to-one to assist people in setting health-related goals and support them to make sustainable changes. This service is free of charge and is open to patients of Mendip GP practices. Come along to your local Talking Café where you can meet new people, make new friends, and chat to a community sign poster about activities, support groups and services in the community.
Wells Talking Café is held at Bishop's Palace every Thursday from 10am-12pm.
Shepton Mallet Talking Café is held at The Art Bank, 13 High Street every Wednesday from 10am-12pm.
Glastonbury Talking Café is held every Monday at St John Baptist's Church from 10am-12pm.
Street Talking Café is held at Fondo Lounge every Wednesday from 10am-12pm.

On Track Group
Come along to these friendly peer led groups where you can join people who are setting goals which might improve their health and wellbeing. Get support and support others, get on track and have a chat over a cup of tea. The On Track groups normally meet every two weeks; please contact us on 01373 468368 to find out specific dates and to register for a place.
